I have the same problem. But for me it was slightly different. She was a teen and I manually added the grim-sprout-trait to her (and never aged her up afterwards). And then I played with different household and then suddenly I saw her with the distorted face. I can't fix it, removing the trait or playing without mods didn't help. It really is a shame for her nice face. :(
The siblings are fine (two kids and one adult). It seems to only affect teens?
