I got tired of waiting on EA to fix this issue and found a mod that attempts to correct it. Interestingly enough, in order to eliminate the facial distortions the mod removes the scythe birthmark. I also managed to track down some old posts in other communities that confirmed my recollection of when this bug started, because it wasn't when the pack released. It started when they updated Nyon Specter to have the Grimborn trait and Grim as his father in the family tree. Just posting that info here hoping it will be helpful at some point in the future when the devs get around to working on this.
