Started the reaper career and I’m supposed to determine the cause of death of a Sim who isn’t on the lot? I’ve tried to locate the deceased sim several times but there isn’t one on the lot

I am having this issue as well. I am on the lot, but there is no deceased Sim; only witnesses. I tried clicking on my Sim to "locate deceased sim," and nothing happened. I have looked high and low several times and cannot find any deceased Sims.
I wasn't able to complete that day, then the next day, the Sim was there but was standing up, even during the investigation. At the end, I couldn't collect their soul because the game was still registering them as alive. Ugh.
I moved this to the bug forum. Bit surprised that there isn't another thread for this. This was fairly common when the pack first came out,

Twice now the game sent me to a lot to reap a sim and there were no dead sims around so I couldn't complete the job. Look everywhere around the lot and nothing. You'd think for such an expensive pack there wouldn't be this many silly issues around
This is happening to me as well. I got the Life & Death expansion pack when it first came out (I preordered) and, for me, there were no issues. But yesterday, for the first time since the pack came out, I tried playing the reaper career again. My sim was able to complete all of her first day training tasks at reaper headquarters, but when I was sent out in the field for training (by travelling via the scrying bowl) the game took me to an empty lot and Kenny, the dummy I was supposed to be practicing my reaping on, was nowhere to be found. The empty lot was in Strangerville and I searched eeeeeeeeverywhere but he was nowhere to be found. No dead bodies either (though according to the game it wasn't supposed to be an actual dead body because I was still in training - it was just supposed to be Kenny. But, yeah. No Kenny). Like another user said, when I tried the self-interaction "locate deceased sim" nothing happened.
Edit to add additional info: I tried reloading the save and, when that didn't work, I tried repairing my game, but no luck.

I am having an issue with Life & Death working with High School Years, where when my sim in the reaper career shows up to the lot, the only witnesses who are both adults immediately leave the lot to go to work, then I fail the work day.
Reaper career: when my sim travels to the lot from the HQ to get a soul, the deceased sim is either not there at all, or nothing happens when I click on the deceased sim. The options such as diagnose, state cause of death, etc. don't appear. The reaper sim can't complete the job. I have no CC or mods and just bought the game 3 days ago.

As for the other issue: Do the Sims lie on the floor or stand ?
