I have this issue in my legacy save, and I don't have life and death. It is always from a previous generation and the only way I can fix it is to play the household where the dying sim lives. Once I've loaded into the household the first thing that happens is the sim dying. It has always been an elder dying of old age.
Same. I have to visit the household of the dying Sim to clear the prompt. I don't own LD either. Always an elder dying of old age.
