This happens to me with my sim's uncle on loading the save or exiting CAS. It's so annoying. He has died at least 10 times by now, always accompanied by the annoying as hell grief system for the entire family. I wish I'd deleted him when I had the chance. I tried to go to his lot and the second it loaded in, his siblings who are still alive immediately got the call he'd died again and started grieving too. I've never hated the grief system more than with this bug.
After playing for a few more hours, when I got the popup about either deleting the sim's inventory or keeping it with his ghost, this time I chose keep it with ghost. Haven't had him die again since. Fingers crossed it sticks this time.
