I have the same issue. After aging up to an adult or an elder, my sim kept reverting to her Young Adult stage. Even after changing her clothes after she aged, when she reverted to Young Adult, her clothes changed back too.
The issue is happening for me even after she died. I set her to be a free-roaming ghost and when she appears on the lot she shows up as a Young Adult who's alive, though still floats around and glows like a ghost. I used to be able to reset the lot by going into Manage Worlds and then playing in the household again, and that would fix it, but that doesn't help anymore.
I know the game updated earlier today for me, so it's on the latest version, and I also play on PC through the EA app.
Really strange and annoying bug.
