I have been having a similar problem.
Gave a sim a makeover in CAS, clothing worn in the game is all the old outfit but when going back in to CAS the new outfits show.
Sometimes this will correct for a short time after the sim goes to bed then wakes up in the correct outfits but will return to old outfits when travelling and only resets again once sim has slept in their bed.
This has been happening for about a month and only happens on one of my sims.
