My sims is randomizing a new outfit after showering. I can delete it in "plan an outfit" but it keeps making new outfits each shower.

I'm not using any mods or cc, but my sim just got a new outfit I hadn't given to her. I noticed it after she travelled to another lot, because she was wearing it. I have no idea when she had got it though. It is a sim that was born into my Pancakes family and who I have been playing for a long time.
I play on PC.
I made her take a shower and she didn't get a new outfit, but when I removed the outfit she got another one. I removed also that and she didn't get a new one.
