This issue has been happening since I purchased the game and seems to come and go. Lately, it's been every time I play and completely game-breaking, making me want to throw my console into traffic.

Can confirm this is still happening with latest patch. The problem its random. It will happen in create a sim can be a couple of mins or not at all and it tends to be create a sim the most. In live mode sometimes you can stop it by switching from live to build mode and back. But a lot of the time only way to fix it is to restart game. It's very disruptive and has been going on for over a year now.
