Hi. I have exactly the same issue, but with a normal sim (not a fairy).
My sim has several reward traits that completely remove the need for the toilet and other basic needs. Free will is disabled, all motives are full, and there are no visible or hidden buffs related to bladder or discomfort.
However, the sim still occasionally gets the toilet thought bubble and the “holding stomach” animation, even though the bladder motive should be disabled.
I tried removing all traits, resetting the sim, deleting caches — the issue still happens.
Fully updated base game with all current packs installed. No mods in use during testing.
Any idea how to fix this on my side?

Since your Sim is not a fairy, I moved your post out into it's own thread.
I though there was another general thread but I have no clue where that went.
What platform are you on ?

I'm on Windows. BTW I also tried saving the Sim to my library and placing them back — it didn’t help.
To investigate further, I installed MCCC to check active and hidden buffs, but found nothing suspicious.
What else can I try? Is there any kind of debug log where I can see what exactly triggers this thought bubble above the Sim’s head?The exact same thing is happening to me. I suspect it's something related to the Enchanted Nature pack and the activation/deactivation of the affliction system. I haven't been able to find anything suspicious with MCCC either.
This is happening to my whole Sim family. Bladder need bar is full, but the toilet thought bubbles pop up around 8 hours for everyone. If I let them pee (although they don't need to), it goes away for several hours (probably 8 but haven't timed it).
