I also had this issue with a non-loyal sim. The only thing I can think of is that this Sim is good traited (plus has the compassionate parenthood reward trait) so perhaps there's some overlap.
For clarity I do play with mods, but this doesn't appear to be an issue with those, especially since I have no custom traits.
