I have a sim in the scientist career and half the time when she goes to work she will put on the barista outfit as she used to work in the career as a teen. My other sim in the fitness career sometimes goes to work in a life and death dress.
I am having the same issue. My doctor is just wearing her fast food out fit instead.
I am also having this issue. My sim was in the barista part-time job as a teen and now is a doctor. Whenever she goes to work in the doctor career, she wears the barista outfit. When I use the career outfit cheat (sims.modify_career_outfit_in_cas) it shows the correct doctor outfit in CAS, but does not update the outfit she is actually wearing at work.
