Aubrey Bloom, from the official Gallery, when being played and turned into a werewolf, she will always have glitched werewolf form with her face being messed up, ears as well, and one eye popping out of the socket.
I do not use any mods, and this always happens when you turn her into a werewolf in game.
She will get normal unglitched form when you turn her to werewolf in CAS, but even then, after you cure her in game, and she gets bitten again, she will gain the same glitched form for some reason.
