Two main factors i can think of to check.
1. Do not use any other fps counter other that origin as they tend to cap at 144.
2. Disable v-sync settings in the Nvidia control panel in the 3D settings section for individual programs.
If it still caps on 144 try this in the command arguments: +fps_max unlimited

? I did i just dont know what V-sync settings OP has tried specifically or what arguments used.
I can only go by the information given to me and check to approach all angles that OP may have missed.

Command lines ain't working. This patch broke the game in a level personally i can't believe it.
Mouse stutters like hell too, can't aim properly and when opening doors game freezes for 0.5sec and resumes.- 7 years ago
You could try disable fullscreen optimisations it usually fixes mouse stutter.
Navigate to your game files and locate the r5apex.exe
then right click and open properties then click the compatability tab
tick the disable fullscreen optimisations and apply.
run the game to test.

Already had that option enabled since the game came out to minimize crashes, issue comes from the game itself. Like i said, didn't change any video settings at all. Hardware & Drivers are the same as a week ago. Probably there's some Engine bug because of that "FPS Cap up to 300" they mentioned on patch notes causing micro-stutters and mice stop responding because of it.
