Keep dying in Three Strikes and can't be revived

What happens when the bug occurs? About 50% of the time in Three Strikes when I am knocked I die within a few seconds even though both team mates are alive. In the very first game of Three Strikes that I played both of my squad mates were knocked. I finished off the 2 teams fighting them and revived both squad mates. Whilst they were restoring their shields etc. a third party attacked. I tried to hold them back whilst my squad mates recovered. I knocked one and then got knocked myself. Within a few seconds I just died and got the message that said something like "Waiting for respawn", My squad mates killed the other 2 enemies and I watched them for about 5 minutes unable to do anything. Not seeing the point of Three Strikes I got bored of waiting and quit. This has happened to my 3 more times since. The last time I was knocked in zone and crawled behind a crate. My squad mates were on full health and close by. They started heading over to revive me and I just died (a few seconds after getting knocked). They even crafted a respawn beacon at a crafting machine for me but it said that they needed my banner. They tried crafting my banner but that was not an option. Why do I keep dying but my team mates just get knocked and wait to be revived. I die about 50% of the time that I get knocked. My team mates never die when knocked unless we all get knocked.
What should be happening instead? In Three Strikes squad mates should be able to revive you. You should not die within seconds of being knocked when both team mates are sill alive and very close to you
Steps: How can we find the bug ourselves? Get knocked in Three Strikes when you squad mates are close and alive. Not sure when else may be required to trigger it.
