The team is aware of this and investigating it. It was added to the Dev Tracker.
This happened to me in PS4 some days ago and I have a theory about why it happens. Actually I uploaded a 3x seconds video in youtube:

Having my weapon back while being downed. My theory is that when Mirage Ultimate skill wears off, it just reequip the weapon (whatever state you are by then), so as I've been downed "invisible", this is what happens after some seconds.
