Revenant shadow mode bugged

Which part of the map or menu were you in? If you don't remember the exact name, please describe the area or what you were trying to do in the menu. I was on top of Diving Board
What were you doing when the issue occurred? E.g. respawning a squad mate at a beacon, using an ability, a squad mate left the game. I was using shadow mode as Revenant and pushing an enemy team on top of Diving Board

How often does the bug occur? Occasionally (10% - 49%)
How severe is your issue? Major impacts to gameplay
What happens when the bug occurs? While in shadow mode as Revenant and my HP drops to 0 from being shot, instead of being teleported back to the totem instead the visual effect, sound effect, and voice line will all play but instead I remain where I was. This got me killed several times in ranked.
What should be happening instead? Revenant should be teleported back to the totem with 50 health or lower depending on what amount of health he activated the totem at.
Steps: How can we find the bug ourselves? I'm not sure how to replicate this, but I will provide all information I can on the specifics of the situation below.
I was fighting a Gibralter and a Lifeline at the time. Lifeline was using a Wingman pistol and a G7 Scout. I'm not sure what Gibralter was using. I am also wearing a Gold Body Shield at the time. I have a Peacekeeper and R99 equipped. Any other info can be obtained from the video attatched.
