Where did the issue occur? In a match
Which part of the map or menu were you in? If you don't remember the exact name, please describe the area or what you were trying to do in the menu. This bug(?) is tied to certain legends. It has nothing to do with the maps or menus.
What were you doing when the issue occurred? E.g. respawning a squad mate at a beacon, using an ability, a squad mate left the game. The issue occurs when activating an ultimate accelerant through the method of simultaneously pressing the LB and RB buttons on the controller.

How many matches had you played in a row before the issue occurred? t occurs every match since it's tied to the ultimate accelerant item and using certain legends.

What happens when the bug occurs? I'm trying to use an ultimate accelerant with Octane, however, around 50% of the time it also activates his tactical ability (stim), which is not what I intend to do.
What should be happening instead? What should be happening 100% of the time when I click the right and left button at the same time is the activation of only the accelerant. Not the tactical ability as well.
Steps: How can we find the bug ourselves? I'm not sure if it's a problem on other platforms, but I can only speak for the PS5 digital console as that's what I play Apex Legends on. I can also only speak for Octane as I hardly play any of the other Legends. To replicate it, you simply have to press the buttons (RB & LB) on the controller at the same time as that's how you normally activate the ultimate accelerant (and ultimate ability if you have it). You'll see that if you do, around 50% of the time you'll also use your (stim) tactical ability along with the ultimate accelerant. This has been tested in-game inside of a match, as well as inside of the firing range. I also tried replicating this bug with the legend Pathfinder inside of the firing range, and he seems to have the same bug, but at a much higher frequency. I'm not sure if it's the same for him inside of a match because I don't play him there, but the steps are the same if you'd like to replicate it.
