Unfair penalty because of Code:Leaf

What happens when the bug occurs? I got Code:Leaf error code during the legend selection, and an abandon penalty for leaving a match. I got Code:Leaf after almost every match, when I'm going back to the lobby, but now I joined to the next match with ingame requeue.
What should be happening instead? Let me rejoin to the match, what I didn't even abandon.
Steps: How can we find the bug ourselves? I have no idea.
