Camera Lock

So a few times while I've been playing my camera will just lock in place and I'm put in third person POV. I went to see how far i could move from where the camera was locked and got about 50 to 60 meters before getting killed and the camera never moved back to my body until after I respawned. Has anyone else had this happen?
