Invisible Weapons
Even when you switch your weapon, your gun is invisible (I believe client-side, however I have seen users without guns as well, just shooting).

I had this as well, happened after I got revived and started trying to fire my weapon immediately, couldn't quit when the match had finished, had to restart whole PC
