Lost all my traits and Objectives progression everything got reset back to zero

To help us look into this issue can I ask you all to please post the following details, thanks.
- Gamertag/PSN ID
- Platform (PS4 or XB1)
- Time Stamp when this was reset, please include date and timzone e.g. 21:32 PST 27/09/17
- Game Mode affected
- What objectives/traits were reset (example All or certain specific ones)
