You're hitting shoot immediately after hitting pass in the hopes you get a one-timer:
If you're passing for a one-timer, don't hit shoot until you know for certain your intended recipient will actually possess the pass. The old-school method of hitting pass and instantly holding shoot will not work as effectively as in previous years and results in taking penalties like that.
