I used to beat the drum of, "of course they test this game" for years but this particular issue proves beyond a shadow of a doubt that these patches are rushed out with no testing.
I can picture a scenario where a group of devs test the new build with icon based passing fixed, and they play ONE game in every mode and determine, 'YEP! Fixed!'.
And now we have a feature that needs to be enabled after every game because QA couldn't bother to test it more than once.
There's also the possibility of good ol' fashioned incompetence. Remember, this is the same dev team that hasn't been able to fix goalie control settings not saving for TWO YEARS.

It's more like probability. Without fail, every update they break something, then spend weeks "trying" to patch that issue, only to break something else. Its like clockwork man, and its unfortunate because EA just doesn't care because at the end of the day, hockey fans will still buy NHL, and addicts will continue to buy HUT packs because that's what they are, addictive, and EA knows this.
