It is confusing, but here's what it is:
Homer has a costume to be a Barbarian (Homer Barbarian)
Barbarian has a costume to be Homer Simpson (Barbarian Homer)
So essentially it's a costume for both characters to be each other. It is part of the storyline. You can't equip them until you complete part 9 of the quest line.
