I would consider that most of my Springfield is reasonably designed these days (although it hasn't always been that way)but I've got a section of town that has become a tetris zone as space has become a premium, I didn't start to design my town till I could afford to buy most of the land so that it was worth planning the design, I simply could not be bothered to keep redesigning my town to start with as I bought more and more land expansion and keep moving stuff around, also have you considered that some of the messy towns you have seen could be feeder towns and the owner aren't bothered about designs just stats for the quick accumulation of cash.
