Only Allows 2 X-Factors on Offense

I have two players on offense with X-Factors. I added an X-Factor to a third player, but when I try to activate him, I get an error pop-up "exceeded the maximum of 3 players with X-Factors enabled for the offensive lineup." clearly that's not the case, as I don't even HAVE three players with X-Factors on my offense, disregarding the one I'm trying to activate. 

Also, all of my abilities and X-Factors are black, not red/white for X-Factors or gold/white for abilities, regardless of whether they're activated or not.
