Flashback permanent black screen
I'm unsure about other platforms but on PS4 Sometimes when pressing the flashback button, it's just a permanent black screen forcing you to restart your game and can sometimes break your game save. It happens pretty frequently for me, I'd say around once or twice every 50% race depending on how much I press or use flashback.
