Still around after patch. Isiah simmons gets a breakout game, gets 3 tfls and then doesn't get an upgrade. No pass/fail message. Like it never even happened.

This is still a super consistent problem btw. It's caused by turning on auto advance in your franchise.
The game stops registering the events once you turn that on.
