2. Yes, if you bleed out and respawn. No if you get revived.
3. They remain until you die or blown up. Switching to a load out from angel crate doesn't make them disappear.

Wait DICE made AT mines disappear on death?- @Popa2caps They've done this a few times at release for some reason. Pretty sure they've always changed it later on to persist after death. I think either BF1 or BFV had them disappear when the player dies.
Not sure why they do this, no one will bother with them if they don't persist after death. @Tank2042Man

Florian has confirmed they are looking into making them persist after death; I'm not entirely sure if it will make it into the December patch or not though
