A process of elimination is probably best here, as @RanzigeRidder2 mentioned. I'm 100% sure I had all of them at some point while playing Rush XL. I had 130 kills there so I would say easy lvl 3 combat. Even if that didn't count I did all 5 of them today on different maps and still nothing.
