Gun model glitch after trying to throw grendade

This happened consistently in the 2 games I played 08/14
I threw a grenade, but hands and grenade disappeared, then didn’t throw.
For the rest of that life, my weapons were either pointed back at my player character (shotgun and pistol), or held in the off hand in low position, with right hand holding an invisible pistol (M4)  

When aiming, it would still zoom and fire.

With the M4, it would aim my empty hand like it was holding a pistol.

With the pistol, it appeared to be partially disassembled.
