Achievements and codex entries still not unlocking

Summarize your bug I have completed the game to 100% and I have only unlocked about 6 achievements from the single player campaign. The codex entries achievements, challenge achievements and game completion achievements have not unlocked. On the companion app it says that the requirement is complete for all of the codex entries but the actual codex is not unlocked. I have tried to clear my game save and do it all again from scratch 3-4 times and it still doesn't unlock the achievements. I have also tried the solutions from other threads where people had the same issue and still, none of them work. It has been like this now for almost 2 years and no update or anything I have tried has fixed any of it
