Make sure you're making no mistakes. While this dog tag is relatively easy, there is a high chance of messing up somewhere. I did get the confirmation that I did everything correctly, but my dog tag wasn't showing. Restarting the game fixed everything.
Restart your game! Just had the same issue that everything was correct and the smoked appeared but back at the main menue I didn´t have it. Restarting game got me the tag in the end.
