Battlefield 4 Campaign Bug Kunlun Mountains Bug

During the campaign mission Kunlun Mountains there are 3 dog tag collectibles that you can collect. After playing thew the mission several times it still only shows that I have collected 2 even tho...

I have the same problem collecting this dog tag, and I have tried it on new saves with and without the 1.02 patch. I was also missing the campaign medal for sacrifising Hannah in the final missiion. I think both of these are stopping me getting the Full arsenal trophy.
