On Origin store, I saw BF4 Premium Edition and Premium Membership both have same price - $49.90
So do they include the base game? If ye, is there anyway to upgrade to Premium without have to pay the Standard Edition again?

Battlefield 4 Premium edition includes the base game and premium pass. Premium Pass for Battlefield 4 gives you access to expansion packs only. So, in your case, if you already bought Standard Edition then I highly recommend you to buy Premium Pass.
