BF4 "Device Hung Error"?

Problem: Every-time I play a BF4 multiplier in any server the game runs fine until the round ends and goes to reload new map/game then an error code pops-up and crashes out of BF4. I have to ctrl-alt-delete out then close the BF4 exe through task manager. It also crashes with the same error message when I fly the attack jet about 10 to 15 seconds in. And it crashes if I quit out of the room before the map changes or ends. I really hope you guys can help I have seen quite a few post with the same problem on the BattleField message boards.

The Error Message reads: DirectX function "Get DeviceRemovedReason" failed with DXGI_ERROR_DEVICE_HUNG ("The device is hung which is typically caused by issues in the graphics driver or aternatively the application").
GPU: "NVIDIA GeForce GTX 660 ", Driver: 34709


What I have done to troubleshoot my issue is:

I have done a clean install of the newest GPU driver twice.

Rolled back to the previous driver with no luck and went back to the latest 

I reinstalled the entire game, plug ins, punk buster, map packs included,

Unparked my cores,

Tried the game on lower and higher settings,

locked my GPU in at factory non-overclocked settings,

did the directx setup in bf4 folder twice.

and I tried the game in both 32bit and 64bit

No changes through any of these adjustments. 

Note:
*I never had this problem before the December update for BF4 can't remember and now it happens EVERY GAME like clock work no exaggeration. I am open to try all suggestions but because of the timing and never having an issue with BattleField before the update I believe that is the culprit.  


Specs:

OS: Windows 7 ultimate 64-bit

Mobo: P8-Z77-V LK
RAM: 8GB 
GPU: GTX Geforce 660 not overclocked recommended settings (updated drivers) (also tried with graphics reduced with no improvement)
CPU: Intel i5 3570k @ 3.40 GHz,
HDD: 1 TB with about 35 GB space remaining
Web Browser: Google Chrome

    Happens to me all the time, even now in 2016, after all the latest and greatest updates to both gpu and other necessary updated drivers. THE STRAAAANGEST THING I FIND WHEN PLAYING BF4 AND THE CRASH HAPPENS IS THAT IT HAPPENS WHEN IM PLAYING REALLY WELL OR * OFF SOME DUDE BY TEABAGGING. HMMMMM..

    Sapphire Radeon R9 295x2 8GB GDDR5
    i7 4790K and using H100i GTX watercooler
    ASUS MAXIMUS VII HERO
    32 GB Corsair Vengeance Pro
    250GB Samsung PRO SSD
    EVGA Supernova ATX 1600 G2
    NZXT Phantom 630
    Windows 10
