Extrapolation offset is how many milliseconds the client (Xbox) is buffering incoming packets it's receiving from the game server.
The more unstable the connection is to the server, the higher this number goes. Which is bad, because it means packets the server is sending to you are getting delayed (buffered in Xbox memory) before being displayed on the TV screen. Obviously this puts you behind in reflex time compared to players with a low extrapolation number.
The reason for extrapolation is to smooth out the movement of other players when packet loss or ping time fluctions (netw vrtn) happen. Otherwise players would teleport around.
You can try connecting to servers in a different region and see if that helps. If it doesn't then it's probably an issue with the home router or ISP.
