Knife swing / Auto Melee error?

Question

Every since purchasing a 360 slim, my characters knife come out automatically whenever I fire my weapon or the enemy points his gun at me. A few round are fired off, then knife come out. I am not even close to him, but I press the fire button, knife come out, switched back over to shooting, knife melee again automatically, and I die. This happens in every game. Why is this happening? I changed controller setting from Default to Veteran, but now instead of a melee, character does an auto prone. It is not the controller because, I purchased a new one and it still happens. Took thumb off of the thumd stick, but it just keeps happening. My 360 slim is fairly new. I can't take this anymore. Please fix this error!!! Is there a fix?

I will provide you several workarounds, please accept as a solution if one of following steps helped you to solve this issue. If not, please respond that we can look for other possible solutions.
To make sure that no corrupt data causing your issue, you should clear your cache:

From the dashboard, click on My Xbox.
Select System Settings.
Select Storage.
Highlight Hard Drive Device.
Press Y to select Device Options, then Clear System Cache.
Reboot your console.

Redownload the updates for the game. NOTE: Not all games have a Title Update File.

From the Dashboard, click on My Xbox.
Select System Settings.
Select Storage.
Select Hard Drive Device by pressing A.
Select Games and Apps.
Select Game Title.
Delete Title Update File.
Restart Game.
Download the update when prompted.

Thanks for the reply. I cleared the system cache about a week and a half ago, downloaded the latest update, and still no joy. It would be great if this issue was fixed on the EA, Dice, or Microsofts end. Any other help would be appreciated.

I will pass your report on to the studio. Could you please try to completely de-install Battlefield 4 from your console in order to re-install the game?
