first of all thank you for your lengthy and well done post. it is much appreciated.
I have the issue of my BF4 soldier stopping randomly while im running or walking, stuttering while running or walking and even moving backwards slowly sometimes while im trying to run or walk. I have tried all of your options. I have tried http://portforward.com/english/routers/port_forwarding/Motorola/SBG6580/Playstation_4.htm just like you recommended and nothing.
This issue has been going on for over a month and obviously it makes a game like this impossible to play. My internet DL and UL speeds are very damn good. I have tried this even with my console as the only LAN cable connected to the router.
PLEASE HELP!

Sorry for the delay in responding to you...what you're describing is what you'll hear people calling "rubberbanding". Those are lag spikes, and this game is plagued by them, lol. It's a server-side issue, not something you can fix on your end...BUT...you could possibly be contributing to the problem if others in your home are online, especially if they are streaming video on Netflix or the like, or if there are downloads or torrents running elsewhere on your network. Generally though, it's a server problem, and the best way to fight it is to play in your own region as much as possible (use the server browser filters, do NOT use quick match) and to leave the server and find another if it's present.
