Hi, I'm using an Xbox one Bluetooth controller (latest model and up to date) on my PC and I encounter the same or similar issue and it end up being the batteries dying, you see when the batteries are low the controller turns on and off randomly and this was causing my games to freeze, I hope you have the same issue so it can be fixed with a simple battery change, good luck man.

You're a lifesaver! I was unable to play Assassin's Creed Odyssey for this same reason. Tried *everything* and nothing was working. Change a couple double A batteries and voila!
