Phantom Trainee and Phantom Initiate

HI, i have completed the phantom trainee and got the paint work but have lost it for some reason and i am unable to complete it again as it shows i have done everything, i have put the pass word in for the phantom intiate and none of the kills are counting, can some please get back to me to help me with this
