This worked!  Only took about 30 minutes to get a rep on chat with me, then about another 30 for them to fix it.  I told them I did all the suggested troubleshooting, needed them to add the entitlements, and gave them the link to this article.  They asked me to hold and when they came back it worked.  Thanks for the info, Tim!
