EA StaffThat sounds pretty much intended I would say. Simply replaying the last mission on hard won't unlock the trophy, you'll have to play every mission/the whole campaign on hard in order to unlock this trophy.
I did replay the whole campaign on Hard, even by pressing "Start New Campaign" and then selecting Hard. I didn't stop and start again at all. That's why I said it even says I've completed the campaign on Hard on Battlelog.
