After the second Januari patch the audio is back to what it was from the beginning, all the sounds is to loud to even hear any footsteps, i don´t know what this patch did to the audio but it´s a mess. sometimes i hear and sometimes i don´t hear footsteps. i think u should take a look into it. because it´s a pain in the ***
Ive set stereo headset modus in audio options and made the sound bearable. Hope it helps you too
Actually, the new sounds are somewhere between what it was a beginning and the last big change - it's not back to what it was at the beginning.

I personally have not have had any issues with it, it's a bit easier to sneak up on people and have not had any irregularities so far in when it's there and when it's not.
like a said, the latest patch must have something to do with audio is accting like this. well, when i don´t hear the enemie and the rest of the sounds takes over the footsteps sound, back where it all began. was the same when the game released. u could not hear anything. before the second january update the sound was perfect for me
You might want to play with the different audio settings in game.
