Looking through the diagnostics section in the DxDiag you attached there appear to be some crashes happening with a few Windows system processes. When you have a chance could you try running the Windows System File Checker to see if that helps? If there's an issue with a missing or corrupted system file that might help fix this. You can find more info on how to run the tool over at the Microsoft Support website.

- @EA_Kent All corrupted systems where repaired, the game is still having issues when I have a controller connected and I'm trying to use a mouse. however, this is only with an official Xbox controller when I plug in a cheap off-brand controller it does ok.

It sounds like it could be an issue with the controller drivers that are built into Windows. Have you checked to see if there might be any updates for the driver through your Device Manager? You can find more info on how to check this over at the Microsoft Support site.
