Dog Tag missing

So, here's the story,  i completed this assignment, make 150 kills with the granade rilfe RMN -50 ( Reccon class ) and im for sure i saw in my screen all of the 3 times when  I completed the first 25, 75 and 150 kills I was given the identification plate, but when I'm in the menu they only appear that they were already completed but they were not delivered to me, I have a photo to prove it.
