Yes, errors joining a match with friends has been happening since the launch on PS4 EU. Happens in duo and also in squad mode. The other player gets an error when the loading starts and the other player from the squad loads into the lobby. The lobby is usually half full so I assume a lot of people experience this.
Also when we play squad mode, we ( 2-3 friends already in a squad) always need to create a new squad after finishing a match. The squad just doesn't fill up with random(s). After creating a new squad random(s) join the squad in seconds and the match begins to load.

I've been watching some Firestorm streams online and people are suggesting to wait about 30 seconds in the squad menu before searching for a new match. The matchmaking failed error doesn't seem to come up as often (or at all) if you wait longer between matches apparently.
