Try testing your connection, this would happen to me in bf4, bfh, bf1 but not yet bfv, but when it would happen to me I would test my connection and it would work. These bugs will probably be fixed on the day one patch on November 20th when the full game releases
Note: Test your connection before a match not while the screen is black
Not a connection issue. It happens randomly when finished with grand operations or an individual conquest or map. Never really during the middle of a campaign.

I had enough of the constant freezes, disconnects and hangs and had them refund the game.
If they fix the netcode (which you'd think wouldnt be a problem after ALL THESE YEARS) I might repurchase the game.
Too goddamm aggravating now
