A few possible solutions:
-Update videocard software drivers
-Defragment or TRIM (SSD) your OS and game drive
-Make sure Windows is not updating in the background during a game (check windows update tab)
-Remove mouse software
-Update mouse software
-Test other USB port
-Re-Calibrate the mouse in the mouse software (calibration is sometimes removed on update or profile switch)
-Lower the ingame mouse speed and up the mouse basic overall movement speed
-Lower BFV graphics settings
-Disable background applications (including mouse software) during a game
-Preform a cleanboot of Windows
thank you for the reply, unfortunately after trying everything on that list I have the same results..
When using 1000 polling rate, my mouse sensitivity is off. when dropping it down to 125hz it is back to normal
I also found that the discrepancy of mouse sensitivity is worse when playing a multiplayer game (vs the test range). for some reason 1000hz polling rate and playing online make the mouse sluggish and about 3-4 inches slower.
I tried changing polling rates in other games and had no change in 360 degree distance with my calculated mouse settings.
Any other suggestions I can try? I have not tried a new mouse, however given that it works in every other game Im hesitant to believe my mouse is the problem.

did more testing..
If i use the command thread.processorcount 6, my CPU usage drops to 80% and my mouse works fine. Default, the thread count is 8 (i have an i7 6700k so that makes sense) but cpu usage bumps to 100%. limiting the thread count to 7 drops cpu usage to 90% but still get mouse issues.
No idea why this started being a problem, but at least that is a fix for it
